An innovative, free, 24-hour, out-of-hours phone health administration advisory service, available to all permanent ADF personnel, serving or on leave in Australia. The advice will focus on where to go for medical treatment in their current location, such as local medical officers, hospitals and pharmacies.

How to arrange admission to The Avenue Hospital:

1. ADF Health Staff to call The Avenue Hospital Nurse Coordinator on ph: 9526 5347 to confirm admission.
2. Go to The Avenue Hospital website www.theavenuehospital.com.au
3. 3. Click on the Greater Melbourne Defence Area Health Services link.
4. Download and complete The Avenue Hospital Pre-Admission Registration Form and Patient History Form
5. Referring Medical Practitioner to download and complete ADMISSION PROFORMA (REFERRAL)
6. Fax completed Pre-Admission Registration Form and Patient History Form along with ADMISSION PROFORMA (REFERRAL) and fax three forms to The Avenue Hospital Nurse Coordinator on fax: 9529 6815.
7. The Avenue Hospital Coordinator will contact physician on call.
8. ADF member transported to The Avenue Hospital. (Please ensure you bring all original forms to hospital upon admission.
